+• <br />40 <br />,f <br />ff , <br />ADDENDUM 43 <br />INDIAN RIVER COUNTY PROJECT 0961.1 <br />COUNTY ROUTE 512 CORRIDOR IMPROVEMENTS <br />ROSELAND ROAD TO 1.95 <br />INDIAN RIVER COUNTY, FL <br />1. POTABLE WATER DESIGN/PERMITTING/CONSTRUCTION SERVICES <br />Preparation of County Route 512 Utility Adjustment Plans for Phase III (Roseland Road to <br />Sebastian River Middle School) and 'Phase IV (Sebastian River Middle School to 1-95) to include <br />potable water system improvements consisting of extensions and/or relocations to County <br />facilities as follows: <br />A. Phase III - Relocation of existing 20" and 12" diameter potable water mains from <br />approximately the Sebastian River Middle School site to just east of the proposed Bridge <br />at the Sebastian River. Along this portion of the CR 512 project, the new roadway grades <br />will cause the existing water main cover to be in excess of six (G') feet. Utility <br />Adjustment Plans will provide for the relocation of the existing potable water main along <br />this portion of the project route to a more reasonable depth for case of` maintenance <br />and/or future connections including placement of the new main on the new bridge <br />structure or beneath the river by a subaqueous crossing. <br />b'ollowing completion of Construction Plans, we shall prepare forms and supporting <br />documents for Submittal of the Construction Plans for acquisition of Potable Water <br />Distribution System permit to construct the project /'ram the Florida Department of <br />Environmental Protection. <br />We shall include within the Contract I)octumcnts for the CR 512 roadway Improvements <br />project specifications indicating the required installation, materials and testing for the <br />potable water system improvements. Rid Documents will be developed for inclusion <br />within the Roadway Improvements project to allow Ior accurate bidding of the Phase III <br />Potable Water System Improvements. <br />Construction services shall be performed for this portion of the Project as described in <br />Attachment "A7— "Construction Services Scope oCServices." <br />'I'he Scope of Services for this project will be expanded to provide for the above described <br />Potable Water Design/Permitting Services listed above within Item IA ibr the 1.ump Sum Pees as <br />follows: <br />Design / Permitting Phase $14,500.00 <br />Construction Phase S 8,400.00 <br />H <br />
