40 <br />40 <br />Addendum #3 <br />06/02104 <br />Page 3 <br />A. Phase III - Relocation of existing 10" diameter force main from approximately the <br />Sebastian River Middle School site to just east of the proposed Bridge at the Sebastian <br />River. Along this portion of the CR 512 project the new roadway grades will cause the <br />existing force main cover to be in excess of6 feet Utility Adjustment Plans will provide <br />for the relocation of the existing force main along this portion of the project route to a <br />more reasonable depth for ease of maintenance and/or future connections including <br />placement of the new main on the new bridge structure or beneath the river by a <br />subaqueous crossing. <br />Following completion of Construction Plans, we shall prepare forms and supporting <br />documents for submittal of the Construction Plans for acquisition of a permit to construct <br />the project from the Florida Department o('Environmental Protection. <br />We shall include within the Contract Documents for the CR 512 Roadway Improvements <br />project specifications indicating the required installation, materials and testing for the <br />force main system improvements. Bid Documents will be developed for inclusion within <br />the Roadway Improvements project to allow for accurate bidding or the proposed Phase <br />III Sanitary Sewer System Improvements. <br />Construction Services shall be performed for this portion of the Project as described in <br />Attachment "A"—"Construction Services Scope ofServices." <br />The Scope of Services for this project will be expanded to provide for the above described <br />Sanitary Sewer Design/Permitting/Construction Services listed above within Iter IIA for the <br />Lump Sum Fees listed as follows: <br />Design / Perinitting Phase $11,400.00 <br />Construction Phase: $ 8,400.00 <br />III. SANITARY SEWER DESIGN/PERMITTING <br />Preparation of Construction Plans for extension of County Master Planned Sewage Force Main <br />System from the Intersection of CR512 & CRS 10 to the North County Wastewater Treatment <br />Plant including the following Scope of Services: <br />We will provide for a Route Survey ol'approximately 18,000 lineal feet, for engineering design <br />purposes for construction of a 12" diameter lorce main along the following route. From the <br />existing 12" diameter force; main in County Route 512, south and cast oil County Route 510, <br />south on Kinks Highway, cast on Hobart Road and north to the North County Wastewater <br />Treatment Plant influent. The route survey will include locating pavement, power poles, mail <br />boxes, driveways, ditches, canals and all visible objects along the route. The survey will also <br />include location of existing underground utilities including the existing water main as well as <br />locates of telephone and fiberoptic lines based on inibrcnatiun to he provided by Utility <br />Companies, HM <br />
