Phase IV: Extensions of potable water main from north to south at approximately <br />twelve (12) locations across existing County Route 512 to provide for ease or future <br />expansion into the Vero Lake Estates 1 Pine Lake Estates Subdivisions. Design of <br />extensions will be compatible with the County's Potable Water Master Plan. It is the <br />intent that the water main extensions be completed by the open -cut method prior to <br />completion of CR 512 Roadway Improvements work. All extensions will be terminated <br />with a valve and lire hydrant to allow the County to flush the system periodically as <br />needed. <br />Following Completion Of Construction Plans, we shall prepare farms and supporting <br />documents for submittal of the Construction Plans for acquisition of Potable Water <br />Distribution System permit to construct the project from the Florida Department of <br />Environmental Protection. Although there will be multiple connections to the existing <br />water main along CR 512, we intend to file only one (1) permit application to the F''DEP <br />to minimize application form preparation and to reduce the County's application fees. <br />We shall include within the Contract Documents for the CR 512 Roadway improvements <br />project specifications indicating the required installation, materials and testing for the <br />potable water system improvements. Bid Documents will be developed for inclusion <br />within the Roadway Improvements project to allow for accurate bidding of the Phase IV <br />Potable Water System Improvements. <br />Construction Services small be performed for this portion of the Project as described in <br />Attachment "A"— "Construction Services Scope of Services." <br />The Scope of Services for this project will be expanded to provide for the above described <br />Potable Water Design/Permitting Services listed above within Iters 113 for the Lump Sum Fees <br />listed as follows; <br />Design 1 Permitting Phase $ 6,200.00 <br />Construction Phase $ 6,400.00 <br />II. SANITARY SEWER DESIGNII)ERMITTINGICf1NSTRUCTION SERVICES <br />Preparation of County Route 512 Utility Adjustment Plans for Phase III ( Roseland Road to <br />Sebastian River Middle School) to include sanitary sewer improvements consisting of extension <br />anchor relocation to existing County Sanitary Sewer force main facilities as follows: <br />
