40 <br />4D <br />i <br />June 21, 2000 <br />Pre -Sid Meeting Agenda For: <br />SEBASTIAN WATER ASSESSMENT PROJECT <br />PHASE 2D <br />UW -97 -13 -DS <br />Description of Project: Construction of approximately 528 L.F. of 12" DIP, <br />16,122 L.F. of 12" PVC, 340 L.F. of 8" PVC, 3,056 L.F. of 6° DIP, 79,024 L.F, <br />of 6" PVC, 80 L.F. of 4" DIP and 3,920 L.F. of 4" PVC water main in the <br />Sebastian Highlands Subdivision -- Phase 2D from existing water mains <br />located on the north side of Collier Creek,, along with fire hydrants and <br />services. The project will include 22-12", 94-6" and 3A" directional bore road <br />crossings. Restore the rights -of -ways as close as possible to the pre - <br />construction condition or better. Construction consists of, but is not limited to <br />mobilization, video/audio recording, utilities exploration, coordination with any <br />permitting agencies, traffic control, trenching, dewatering, installation of pipe <br />with fittings and all appurtenances, restrainers, testing, disinfection (includes <br />providing temporary jumper or another approved method for disinfection and <br />flushing), soil compaction, pavement restoration, irrigation repair, sod <br />restoration, providing as -built drawings, demobilization, insurance bonds, <br />cleaning, etc. <br />2. The water main will be constructed on the north side of Collier Creek, east to <br />Louisiana Avenue. One positive aspect of the job is there will be five <br />additional projects of similar nature in the same vicinity following in <br />approximately six month intervals. <br />3. All DIP must be wrapped in polyethylene liners unless contractor can provide <br />an acceptable corrositivity test report indicating that liner is not required. <br />4. Existing grade of swales must be measured at 100' intervals, relative to the <br />elevation of road crown, prior to commencement of construction. <br />5. A City of Sebastian Right -4f -Way and Road Opening Permit is required for all <br />road cuts on city roads. One -$1,500 deposit must be paid by the contractor <br />for all proposed road cuts. In certain cases as indicated on plans, road cut <br />restoration will require entire intersections to be overlaid with asphalt.
