Description of Services to be Provided <br />Working with Vero Beach —Indian River County Recreation Department to collaborate in offering <br />Parenting Tools to parents of children. <br />Working in conjunction with faith community leaders, child care directors, school administrators <br />and other organizations, the Provider will identify time and place far regularly scheduled Parenting <br />Tools sessions throughout the county. All groups are to be open to the public. Additional <br />responsibilities include organization and oversight of training of volunteer leaders. <br />These services can also be provided in the neighborhood, school, workplace, or clinic, wherever the <br />concerns, priorities, and resources of the parent or caregiver can best be met. <br />VII. Fiscal and Programmatic luta Collection and Reporting <br />A. Financial --The Provider will provide a financial report to the Coalition on Healthy Start funds <br />expended by category. This report will be submitted by: <br />October 31, 2000 <br />covering: July 1, 2000 — September 30, 2000 <br />January 31, 2001 <br />covering: October 1, 2000 --;December 31, 2000 <br />April 30, 2001 <br />covering; January 1, 2001 — March 30, 2001 <br />July 31, 2001 <br />covering: April 1, 2001 -- June 30, 2001 <br />B. Services -- The Provider will report services completed in the project. The Provider is <br />responsible for the accuracy of this data. <br />Working in conjunction with faith communities leaders, child care directors, school <br />administrators and other organizations, determine time and place for regularly scheduled <br />Parenting Tools throughout the county. All groups arc to be open to the public. Additional <br />responsibilities include organization and oversight of training of leaders. <br />The Provider will provide a quarterly report to the Coalition which addresses progress toward <br />achieving the performance measures included in Section IX below. <br />This report will be submitted within ten working days following the end of the month. The <br />Coalition will monitor services provided by the Provider through their reports. <br />VIII. Insurance <br />The Provider will provide adequate liability insurance coverage on a comprehensive basis and to <br />hold such liability insurance at all times during the existence of this contract and any renewal(s) and <br />extension(s) of it. Upon execution of this contract, the provider shall furnish the Coalition written <br />verification supporting bout the determination and existence of such insurance coverage.
