40 <br />4D <br />M <br />IX. Evaluation and Monitoring <br />A. Annual Performance Measures -- The Provider's manner of service delivery will be evaluated <br />using the following standards: <br />Outcome Indicator — Goal of a minimum of six (6) Parenting Tools participants at each <br />site. <br />Process Indicator— Write and distribute at least one (1)notification of gatherings <br />for distribution through local newspaper, PTA newsletters, housing developments, <br />provider brochures and other media which parents receive. <br />Outcome Indicator -• Sixty percent (60%) of Parenting Tools participants agree that <br />Parenting Tools is helpful to them. <br />Outcome Indicator -- After attending six Parenting Tools gatherings, twenty-five percent <br />(25%) of participants say they "almost never" or "never" yelled or shouted at their children. <br />Outcome Indicator -- Fifty percent (50%) report that after attending six (6) Parenting Tools <br />gatherings, they would "never" or "almost never" hit or spank their child. <br />Outcome Indicator -- Seventy percent (70%) of participants agree that they made positive <br />changes in their parenting and in themselves as a result of attending six(6) Parenting Tools <br />gatherings. <br />Process Indicator — Maintain five (5) existing Parenting Tools sites, with <br />associated volunteer leaders, <br />Process Indicator — Expand to an additional five (5) Parenting Tools sites, with <br />associated volunteer leaders. <br />Process Indicator — Offer two (2) additional Parenting Tools trainings for <br />volunteer leaders. <br />Process Indicator — Recruit and train an additional five (5) additional Parenting <br />Tools trainings for volunteer leaders. <br />Process Indicator -- Offer a consumer satisfaction survey to Parenting Tools <br />participants twice a year to record outcomes. <br />B. Periodic Monitoring -- The Recreation Department will receive a site visit at least once every <br />six months year to determine compliance with Healthy Start performance measures adopted by <br />the Coalition. The format of this site visit will include at a minimum. a record review of all <br />participating Parenting Education sites, an examination of the Provider compliance with <br />performance measures included in Section VIII; and an analysis of rite strengths, weaknesses <br />and planned changes for the program.
