FA <br />i <br />ADDITIONAL SERVICES ADDENDUM <br />FOR <br />THE HAZARDOUS WASTE MANAGEMENT PROGRAM <br />AND <br />MONITORING & MAINTENANCE OF THE WETLAND MITIGATION SITE <br />FOR THE <br />INDIAN RIVER COUNTY SOLID WASTE DISPOSAL DISTRICT <br />Eeotech Consultants, Inc, (ETC) and Indiana River County (County) agree to amend the <br />Standard Form Agreement for monitoring services to include assisting the County will the <br />hazardous waste management program associated with the operation of the solid waste landfill. <br />In general, ETC will perform the tasks previously assigned to the Environmental Chemist and <br />Hazardous Waste Technician positions at the landfill. Presently, both of these positions are <br />vacant, however, the County continues to receive and handle hazardous wastes at the landfill and <br />transfer stations. ETC will assist the County on a monthly basis following the terms defilted <br />below in Tasks 1-3. The services associated with the contract will be divided into four tasks as <br />follows: <br />ETC will perform monitoring and maintenance services for the approximately 12 acre mitigation <br />site located adjacent to Old Dixie Highway between 22"d and 23'a Streets in Vero Beach <br />(location and identity of the mitigation site as described in the Florida Department of <br />Environmental Protection Permit Number MS31-247323). The services to be performed by ETC <br />shall be those described by Tasks 4-7 presented below. ETC shall commence work on these <br />services within 30 days of receipt of the notice to proceed from the County. If the County elects <br />to have ETC remove the cattail plants from the mitigation site, then the County shall place a <br />suitably sized garbage bin at the mitigation site for disposal of this plant material. <br />The County may terminate this Additional Services Addendum partially or completely for any <br />reason, upon 34 days written notice. <br />SCOPE OF SERVICES HAZARDOUS WASTE MANAGEMENT PROGRAM <br />Task 1. Environmental Audit ('Staffing Assistance) <br />ITC will audit the storage, handling, and reporting process currently implemented by the <br />COUNTY at the landfill and each transfer station, within 90 days of Notice to Proceed. The <br />primary purpose of this audit will be to review compliance of COUNTY operations with permit <br />and OSHA requirements and conditions. A draft report will be provided to the County for <br />review, after which, a final report outlining the findings and recommendations will be submitted <br />to the County. This report will be provided (in hard copy and electronic formats) to the County <br />within 30 days of completion of the County receipt of the County review cotr+ments. <br />11age 1 of 5 <br />
