All boxes on the survey forms were not checked by the facilities <br />and some had footnotes. The forms are attached for review as the <br />Board deems appropriate. The survey indicated the following: <br />1. -Is your Facility/Organization satisfied with the current <br />wheelchair service being provided to you? <br />2. <br />3. <br />4. <br />5. <br />Yes 5 <br />No 5 <br />Has your Facility/Organization experienced any delays in <br />transport? <br />Yes 9 <br />No 3 <br />Does you(sic) Facility/Organization feel there is a need for <br />Able Health Services to provide wheelchair service for private <br />pay? <br />Yes 13 <br />Does your Facility/Organization feel that there is a need for <br />another wheelchair provider in Indian River County? <br />Yes 13 <br />No 0 <br />Would your Facility/Organization like to have another service <br />to choose from for your Private Pay wheelchair clients? <br />Yes 13 <br />No 0 <br />3) Letter dated June 6, 1996, from Vincent Cacciatore, <br />Administrator of Palm Garden located at 1755 37th Street, Vero <br />Beach. Mr. Cacciatore stated he has been a nursing home <br />administrator in Vero Beach for over 10 years. He advises there is <br />no doubt in his mind that there is a need for another private pay <br />wheelchair transport company in the County. It should be noted <br />that the EMS Director was notified on September 12, 1996, that Mr. <br />Cacciatore had been replaced by Mr. Stan Weir as Administrator at <br />Palm Garden. <br />4) Letter dated August 1, 1996, from Thomas Fritz, <br />Transportation Director for the Council On Aging and Community <br />Transportation Coordinator for Indian River County located at 694 <br />14th Street, Vero Beach. Mr. Fritz stated that Able is not under <br />coordination contract with the Council On Aging and alludes to a <br />recent meeting of the Transportation Disadvantaged Local <br />Coordinating Board wherein it was agreed that there was no need for <br />additional transportation operators at this time. He concluded his <br />letter with comments relating to Able's Class "B" certificate which <br />is not at issue at this time. <br />5) Letter dated August 5, 1996, from Sebastian City Manager <br />Thomas Frame advising that he had reviewed the Able application and <br />discussed the matter with representatives of the Sebastian River' <br />Medical Center. He noted it would appear that this company is <br />capable of providing adequate service to the Sebastian area. He <br />concluded his letter by stating that based on his findings, he <br />would leave the determination fully up to the EMS Director as a <br />recommendation to the Board of County Commissioners for final <br />determination. <br />6) Phone call on August 8, 1996, at 11:10 A.M. from Bruce <br />Miller of Royal Palm as a follow-up to his letter of May 6, 1996. <br />He stated he was worried about what was going to happen with <br />28 <br />September 24, 1996 `) <br />
