Page 1 of 2 <br /> CHANGE ORDER BACK-UP DOCUMENTATION <br /> FIN No.: 436379-1-54-01 & 436379-1-54-02 Contract No: IRC-1505 <br /> Change Order No.: 03 Amount $ 814,277.14 and 0 Days <br /> Title of Issue <br /> Construction of 53rd Street Signalized Intersection at 66th Avenue in accordance with Simmons & <br /> White (Engineer-of-Record) plans Signed & Sealed dated September 28, 2022. <br /> I. Entitlement Analysis <br /> The purpose of this Change Order is to provide means to compensate the Contractor to construct a <br /> new signalized intersection on 66th Avenue at 53rd Street. This scope of work was not included at <br /> time of original bid. <br /> II. Time Analysis <br /> The Contractor is not pursuing a time extension to complete this additional work. <br /> III. Engineer's Estimate <br /> a. Basis of Estimate <br /> The Engineer's Estimate was based on current State-Wide-Average (SWA) multiplied by the <br /> Engineer-of-Records' plan quantities for this new scope of work. Considering the increase in <br /> unit cost is appropriate as the decision to proceed with this extra work occurred at this time <br /> and the Contractor was unable to proceed with procurement of the necessary components <br /> earlier. To this we added Mobilization & Maintenance of Traffic(MOT)as a percentage based <br /> on original contract. This resulted in an Engineer's Estimate totaling $827,689.97. <br /> b. Estimate <br /> The Engineer's Estimate to add a signalized intersection at 53rd Street resulted in the net <br /> increase to Contract in the amount of $827,689.97. This price includes all mark-ups and no <br /> bonding. <br /> Refer to Appendix"A"for a breakdown of the Engineer's Estimate and backup documentation. <br /> c. Contractor's Quote <br /> The Contractor's quote submitted a Change Order in the amount of Eight Hundred Fourteen <br /> Thousand Two Hundred Seventy-Seven and Fourteen Cents ($814,277.14). <br /> Refer to Appendix "B"for a breakdown of the Contractor's cost proposal. <br /> d. Final Disposition <br /> After comparing the Engineer's Estimate of $827,689.97 to the Contractor's price of <br /> $814,277.14 the Contractor's proposal is less than the Engineer's Estimate by $13,412.83. <br /> After review of the Contractor's proposal the County has agreed to the Contractors price of <br /> $814,277.14 has been accepted. <br /> IV. CRS Coding <br /> 503 Change resulting from engineering decision (use specific reason in lieu of this when possible) <br />
