CI <br />r <br />40 <br />1) DEP Solid Waste Division site inspection reports for period between June 15, 2000 to <br />June 15, 2001. <br />2) Any records of any odor complaints received by either the SWDD or the County <br />pertaining to the Landfill. <br />3) Total waste received (tons or cubic yards) at the landfill during this period (and source <br />of this information). <br />4) Information on whether or not additional Landfill capacity was permitted in the <br />reporting period. <br />5) Amount of waste (tons) and types combusted during the reporting period in the Air <br />Curtain Incinerator, and source (monthly log) of this information. If no waste has been <br />combusted, last date that the Incinerator was used, whether or not it has been shut <br />down, and whether or not it will be started up or used in the future. <br />b) List of equipment (insignificant emissions units) used at the Landfill in the reporting <br />period, and currently being used. 'Examples include: yard waste tub grinder, C&D <br />Rome Mauler, trommel, shredder, wheel remover, and dozers, loaders, compactors, etc. <br />CDM wail provide the list used in the Title V Permit Application, and request that <br />SWDD indicate changes and revisions. <br />7) Information on whether open burning occurred at the Landfill during the reporting <br />period. <br />8) Verification that disposal of appliances containing refrigerants (regulated ozone- <br />depleting substances) is being performed with equipment certified pursuant to 40 CFR <br />82.161, and in accordance with training requirements and procedures in 40 CPR 82 <br />Subpart F. This will likely require getting copies of certifications from the contractor <br />performing this work. <br />9) Any information in maintenance logs pertaining to dust suppression activities. <br />Examples include watering of piles and unpaved areas, and revegetation of closed <br />landfill areas. <br />CDM will prepare a preliminary draft of the Statement of Compliance Table that highlights all <br />information needed, as well as update the summary list above. CDM will work with the SWDD <br />to either identify potential sources of any difficult -to -obtain information, or to develop <br />defensible estimates, wherever necessary. <br />TASK 5.1.2 --- PREPARATION OF DRAFT AND FINAL STATEMENT OF COMPLIANCE <br />The Statement of Compliance will be a large matrix table listing every Title V Permit condition, <br />and containing columns for the following information for each condition: <br />Methods or other information used to determine the compliance status, <br />Compliance status for this permit condition, <br />rb3443.doc 13-5 <br />
