This is a statement, signed <br />by a representative of the SWDD, that the compliance information being submitted is true, <br />accurate and complete. <br />CDM will complete the preliminary draft by filling in the information compiled as part of Task <br />5.1.1. CDM will submit this draft Statement of Compliance to SWDD for comments. When <br />comments are received, CDM will prepare a final Statement of Compliance for SWDD review <br />and signature by the Responsible Official. CDM will then submit the signed final Statement to <br />DEP. CDM will meet with TRC up to six times to discuss issues and to provide needed <br />coordination. CDM will also respond to FDEP correspondence regarding the Title V issues. <br />CDM's proposed schedule for Task 5.1 is; <br />r CDM submits preliminary draft Statement of Compliance matrix table showing needed <br />information, and updated data needs list, to SWDD by April 15, 2001. <br />■ CDM submits the final Statement of Compliance to FDEP by June 1, 2001. <br />TASK 5.2 - PREPARATION OF STATEMENT OF COMPLIANCE FOR YEAR 2000 <br />CDM will prepare the Title V Annual Operating Report (AOR) and Annual Emissions Fee form, <br />both due to DEP in March, 2001. These reports will be based on reports prepared previously <br />updated with year 2000 operational and compliance information.
