Vero Beach, FL. The <br />purpose of this Survey is to comply with the EPA NESHAP regulations that require the <br />Building owner to conduct an asbestos survey to identify asbestos containing materials <br />prior to renovation activities. This building interior is to be renovated and the renovation <br />and construction debris is to be removed from the property. <br />On September 18, 2000 H. William Levy, Certified Asbestos Inspector 82223-3036 <br />conducted this survey and sample collection, <br />'This report is based on the building materials observed at the Building on the date of the <br />,survey. The areas determined to be inaccessible at the time of this Survey are attics, <br />closed stair wells and underground utilities. Ouring the initial walk through and before <br />collecting any samples, distinct homogeneous materials were identified and a sampling <br />strategy, within the requested scope of services, developed. Representative samples <br />were randomly collected from homogeneous materials, which are similar in appearance, <br />taxture, and color; and have the same installation date, use and function, DRrwall wall <br />and ceiling system samples were collected as a composite of the material. <br />An independent EPA NVLAP accredited laboratory completed sample analysis. The <br />EPAMESHAP recommended method for analysis, polarized light microscopy (PLM) and <br />tiisuaI estimation was used to determine the type and percentage of asbestos. <br />��nstruct on- <br />Th•e single level 4,000 sq. ft. masonry building with asphalt shingle roofs was reported <br />to have been constructed in the late 1960's. The building interior has been renovated, <br />Building exteriors are painted stucco on concrete block. Interior walls are drywall, wood <br />paneled or painted stucco covered masonry. Ceilings are pained drywall, Floors are <br />painted concrete, resilient linoleum, 12" x 12" VAT, terrazzo or covered with carpeting.. <br />Coors and windows are wood frame. <br />
