4b <br />i <br />40 <br />18.0 PUBLIC DISCLOSURE STATEMENT <br />12.3 Conflict of Interest: Any entity submitting a bid or entering into a contract with <br />the OWNER shall disclose any relationship that may exist between the contracting entity <br />and a County Commissioner or County employee. The relationship with either must be <br />disclosed as follows: <br />12.1.1 Father, mother, son daughter, brother, sister, uncle, aunt, first cousin, <br />nepheav, niece, husband, wife, father-in-law, mother-in-law, daughter-in-law, <br />son -in -lax, brother- in-laav, sister-in-law; stepfather, stepmother, stepson, stepdaughter, <br />stepbrother, half brother, half sister, grandparent, or grandchild <br />12.2 The term "affiliate" includes those officers, directors, executives, partners, <br />shareholders, employees, members, and agents who are active in the management of the <br />entity. T he disclosure of relationships shall be a sworn statement made on a County <br />approved form. (Form provided in the Forms Section of this Bid). Failure to submit <br />form may be cause for rejection of bid. <br />13.0 Anything in the contract documents notwithstanding, in no event shall the <br />OWNER or CONTRACTOR be liable for special, indirect, incidental or consequential <br />damages, including commercial loss or lost profits, even if either party has been advised <br />of the possibility of such damages. <br />
