Mehta, P.E. <br />17 June 2022 <br />Page 2 <br />Phase 1— General Consulting/Meeting Support/Project Management <br />Under this phase, Geosyntec performed project planning and management responsibilities, such <br />as kick-off and pre -application meetings and correspondences with SWDD and Florida <br />Department of Environmental Protection (FDEP); invoice review and preparation; project <br />coordination; and project administration. <br />Phase 2 — Preparation of Solid Waste Renewal Permit Application <br />Under this phase, Geosyntec prepared and submitted the solid waste renewal permit application <br />for the C&D debris disposal facility to FDEP on 9 May 2022 and assisted SWDD and IRC to pay <br />the required permit fee online. <br />Phase 3 — Response to Regulatory Comments <br />Under this phase, Geosyntec responded to request for additional information (RAI) by FDEP. A <br />final permit was issued by FDEP on 1 June 2022 and Geosyntec reviewed it for consistency with <br />the previous permits and to make sure there were no new specific permit conditions. <br />BUDGET <br />The approved budget for Work Order No. 15 was for a lump sum amount of $38,993.00. The <br />total invoiced amount, including the final invoice is $38,993.00.
