The Recipient's Agreement Manager is responsible for monitoring the performance of <br />this Agreement's terms and conditions and will serve as the Recipient's liaison with <br />the Department. As part of the duties, the Agreement Manager for the Recipient shall <br />provide the Department with all reports in accordance with Section 15, Reports, of this <br />Agreement, as well as any other required reports or documents under this Agreement. <br />D. In the event that different Managers or addresses are designated by either party after <br />execution of this Agreement, a. notice of the name, title, and address of the new <br />Manager will be provided to the other party in writing. Such changes do not require a <br />formal written amendment to the Agreement. <br />(4) TERMS AND CONDITIONS <br />This Agreement consists , of this State -Funded Agreement and all incorporated <br />attachments and exhibits, which set forth .the entire understanding of. the parties and <br />supersede all prior agreements and understanding related to the subject matter thereof. <br />(5) EXECUTION <br />This Agreement maybe executed in any number of counterparts, each of which, when <br />executed and delivered, shall constitute a duplicate original, but all counterparts together <br />shall constitute a single agreement. . <br />(6) MODIFICATION <br />Either party may request a modification of provisions of this Agreement via a formal <br />amendment, which shall be valid only when in writing, signed by each of the parties, and <br />attached to the original version of this Agreement. <br />(7) SCOPE OF WORK <br />The Recipient shall perform the work in accordance with Attachment A, Scope of Work <br />and Budget, of this Agreement. <br />DMS -P1-25-07-17 Page 3 of 18 <br />
