6. Program Crediting and Signage <br />The Grantee shall display the appropriate pumpout symbol on facilities, such as pumpout and portable toilet dump <br />stations, or on printed material or other visual representations relating to Project accomplishments or <br />education/information (50 CFR §85.43 and 50 CFR §85.47). <br />A. If specified in Attachment 3, Grant Work Plan, the following signage is required: <br />i. One (1) three-foot (3') by four -foot (4') sign of the International Pumpout Symbol to be located on <br />a dock or on land facing the waterway and clearly visible to boaters. <br />ii. One (1) informational sign, to be posted in a clearly visible location adjacent to the pumpout <br />equipment, which must state fees, restrictions, hours of operation, operating instructions, an <br />operator name and phone number, emergency phone numbers for reporting service problems, and <br />include the following statement: <br />"Funded in part by the U.S. Fish and Wildlife Service, Clean Vessel Act through the Florida <br />Department of Environmental Protection." <br />B. If required by Attachment 3, Grant Work Plan, all other printed materials or visual representations related to <br />the Project, including education and instructional materials shall include the following statement: <br />"Funded in part by the U.S. Fish and Wildlife Service, Clean Vessel Act through the Florida Department of <br />Environmental Protection." <br />7. Project Required Submittals and Requirements <br />The following documents are required submittals under this Agreement. Failure to provide any of the following in <br />the timeframes provided may result in denial of reimbursement request. These provisions also represent <br />requirements under this Agreement that must be complied with for the term of this Agreement. <br />A. The Grantee shall submit copy(s) of any executed subcontracts for the completion of this project as <br />described in Attachment 2, Special Terms and Conditions, paragraph 11. <br />B. In addition to the detailed supporting documentation required for reimbursement, as described herein, the <br />Grantee shall, with the final reimbursement request, submit the Quarterly Pumpout Log, as described <br />herein, when one or both of the following conditions apply: <br />1. When the Project includes the purchase and/or installation ofpumpout equipment, the Grantee is <br />responsible for submitting the Quarterly Pumpout Log for a period of five (5) years. The five (5) <br />year reporting period begins upon either the complete installation of all pumpout equipment or <br />the Department's Grant Manager's acceptance of the final deliverables and submittal of the final <br />invoice, whichever comes later. The Quarterly Pumpout Log is due every quarter thereafter for <br />the following five (5) years. <br />2. When the Project includes operations and/or maintenance and repair, the Grantee is responsible for <br />submitting the Quarterly Pumpout Log beginning upon execution of this Agreement, specifically <br />at the first quarter of operations, and continuing each quarter thereafter through the expiration of <br />this Agreement. <br />Quarterly Pumpout Logs must be retained as backup documentation for a period of five (5) years from the project <br />end date. <br />Rev. 10.31.25 Attachment 6 <br />Page 2 of 3 <br />
