Chandler <br />County Administrator <br />FROM: Ann Rankin - 7� <br />Benefits/Payroll Administrator <br />DATE: May 19, 1999 <br />SUBJECT: Dental Plan Specifications <br />Staff requests consideration of the following information at the May 25, 1999, regular meeting of the <br />Board of County Commissioners. <br />Background <br />At the regular Board meeting on March 23, 1999, the Board of County Commissioners approved <br />a proposal for dental insurance from Guardian Life Insurance Company. <br />Recommendation <br />Staff is requesting the Board execute the Specifications for a Non Medical Plan of Group Insurance, <br />also knows as, Master Application. <br />ON MOTION by Commissioner Ginn, SECONDED BY <br />Commissioner Stanbridge, the Board unanimously authorized <br />the Chairman to execute the Specifications for a Non -Medical <br />Plan of Group Insurance aka Master Application of The <br />Guardian Life Insurance Company, as requested in the <br />Memorandum. <br />COPY OF DOCUMENT IS ON FILE <br />IN THE OFFICE OF TAE CLERK TO TIRE BOARD <br />14.A.
