SECOND MODIFICATION TO DENTAL INSURANCE <br />PROPOSAL - GUARDIAN LIFE INSURANCE COMPANY <br />The Board reviewed a Memorandum of May 19, 1999: <br />TO: The Board of County Commissioners <br />FROM: u,C/ William G. Collins II - Deputy County Attorney <br />DATE: May 19, 1999 <br />SUBJECT: Modification of Dental Insurance Proposal <br />On March 23, 1999 the Board of County Commissioners approved a proposal for <br />dental insurance from the Guardian contingent on a minimum enrollment <br />participation of 35% of County and/or constitutional employees. <br />The Guardian has recently contacted us and has proposed to amend its <br />proposal to reduce the minimum participation to 29% of Commission and/or <br />constitutional employees. Excluding the Sheriff's Office, 29% of Commission <br />and constitutional employees (236 employees) have applied for enrollment in the <br />dental insurance program. Guardian is willing to make this modification because <br />of the short term of this enrollment period, i.e., there will be another enrollment <br />period in August. <br />RECOMMENDATION: <br />Authorize the Chairman to execute the attached "Second Modification to Dental <br />Insurance Proposal of the Guardian" reducing the minimum participation <br />requirement to 29% of eligible employees. <br />(CLERK'S NOTE: Additional information giving Status of Dental Benefit <br />Participation has been placed on file with the backup in the office of the Clerk to the Board.) <br />ON MOTION by Commissioner Adams, SECONDED BY <br />Commissioner Ginn, the Board ,inanimously authorized the <br />Chairman to execute the Second Modification to Dental <br />Insurance Proposal of the Guardian reducing the minimum <br />participation requirement to 29% of the eligible employees, as <br />recommended in the Memorandum. <br />MAY 259 1999 <br />SECOND MODIFICATION IS ON FILE <br />IN TAE OFFICE OF THE CLERK TO THE BOARD <br />46 <br />go 0 <br />
