OW <br />Vele <br />IMO <br />SPECIAL CONDITIONS FOR PERMIT NO. 14-14 <br />FOR <br />EXTENSION OF CONCRETE BOX CULVERT IN <br />LATERAL "E" (20TH AVENUE) CANAL AT 16TH STREET <br />(SUB -LATERAL E-2 CANAL) INTERSECTION <br />(1) This permit is issued based on plans for County Project No. 0225, prepared by <br />Bridge Design Associate, Inc., dated July 1, 2014; and Roadway Intersection <br />Improvement Plans prepared by Indian River County Engineering Division, <br />signed and sealed by Michael D. Nixon, P.E., dated July 10, 2014. <br />(2) Drainage within Lateral "E" (20th Avenue) and Sub -lateral E-2 (16th Street) <br />Canals shall be provided and maintained at all times during construction of <br />proposed concrete box culvert. <br />(A) For that section of 6' x 6' box culvert extension lying south (upstream) <br />of 16th Street (Sub -lateral E-2 Canal) the Permittee/Contractor shall <br />provide a total maximum by-pass pumping capacity of 16,000 gallons <br />per minute on-site and operational (as may be needed) at all times. <br />(B) For that section of 6' x 9' box culvert extension lying north <br />(downstream) Sub -lateral E-2 (16th Street) Canal, the <br />Permittee/Contractor shall provide a total maximum by-pass pumping <br />capacity of 32,000 gallons per minute on-site and operational (as may be <br />needed) at all times. <br />(3) Any and all proposed dewatering plan for this project shall be approved in <br />advance of proposed project construction by Indian River Farrns Water Control <br />District Superintendent of Operations, David E. Gunter (Phone Number — 772- <br />562-2141). <br />(4) All proposed sheeting, berms, or other temporary blockage within the Lateral <br />"E" (20th Avenue) or Sub -lateral E-2 (16th Street) Canals shall be coordinated <br />with Superintendent David E. Gunter on a day-to-day basis throughout the <br />construction phase. <br />In the event a severe rainfall event occurs, or is expected to occur, resulting in <br />canal stage/flow that exceeds the allowable upstream by-pass pumping <br />capacity, the Permittee/Contractor shall be able to immediately remove all <br />temporary blockages within the canal, and provide a full capacity flow in the <br />canal as may be required in an emergency condition. <br />(6) All perspective bidders of this project shall be made aware of the applicable <br />standard provisos and special conditions of this permit. Superintendent David <br />E.
