(772) 569-9208 <br />16 October 2014 (revised 30 March 2015) <br />TO: Jason Pykes <br />Barth Construction <br />1717 Indian River Blvd. <br />Suite 202A <br />Vero Beach, Florida 32960 <br />RE: Proposal for Part 1 Services on the Existing Facility <br />SITE: Station #1: 1500 Old Dixie Highway, Vero Beach, FL 32960 <br />Architect's Project #100114VB <br />Dear Jason, <br />In accordance with the Part 1 Agreement between Indian River County and Barth Construction <br />acting as Design/Builder, and specifically Article 1.3, we are herewith submitting the following <br />request for work authorization to provide Architectural Services to renovate the existing facility. <br />A. Basic Services <br />Our evaluation and documentation shall include the following: <br />I. Establish Programming and Scope data as necessary to achieve a complete <br />description of work to be performed. See initial Programming/Scope description <br />attached dated 16 October 2014. <br />2. Visit the project site, take digital photos, actual dimensions, and meet with the <br />end-users to note all concerns regarding the building and site conditions. <br />3. Research archival and electronic plans available as provided by the Owner and in <br />the absence thereof, field visit the site and document current conditions in <br />AutoCad format. <br />4. Provide Schematic Design and 100% Design Development Documents to depict <br />and define areas of demolition, renovation, remodeling or new construction for <br />the purposes of quantifying areas, volumes, structural systems, and basic <br />mechanical/electrical infrastructures within the primary building. Provide an <br />Outline Specification. These documents can be utilized by Barth Construction to <br />determine the first estimate of construction costs. <br />Page 1 of 3 <br />
