Exhibit A <br /> Continuing Contract Agreement <br /> Work Order No. 2 <br /> Scope of Work <br /> Indian River County Utility Services <br /> West Regional WWTP 2015/2016 PERMIT RENEWAL <br /> The current Indian River County IRCUD/West Regional WWTF Wastewater Facility - <br /> Permit No. FL0041637 will be expiring on April 11, 2016. The renewal application will be <br /> due on or before October 11, 2015. Failure to submit the application 180 days prior to <br /> the expiration date of the referenced permit will result in enforcement action. Atkins has <br /> contacted FDEP regarding the renewal of permit for the referenced facility and they have <br /> indicated that the following forms and/or information will be required: <br /> 1. FDEP Form 1 <br /> 2. FDEP Form 2A <br /> 3. O&M Performance Report <br /> 4. Process Flow Diagram <br /> 5. Site Plan <br /> 6. Location Map <br /> 7. Binding Agreements and Documentation of Controls on Individual Users of <br /> Reclaimed Water <br /> 8. Reclaimed Water or Effluent Analysis Report <br /> 9. Since the current flows to the WWTP are below 50% of the design capacity, <br /> according to rule 62-600.405(4)(C) a capacity analysis report is in not required, <br /> however based on discussion with FDEP, they would require a certified <br /> statement to this effect. ATKINS will address this either in the signed and sealed <br /> cover letter or in the O&M performance report. <br /> Based on the above ATKINS has prepared the following scope. Details of the data <br /> requirements are attached in Attachment A. <br /> Task 1 Project Kick off and Data Collection <br /> Task 1 requires gathering the data and updating Form 1, Form 2A and the permitting <br /> package. PBSJ will work directly with IRC and WRWTF staff to obtain the required data <br /> and complete the required forms and reports. A Project Kickoff meeting must be held <br /> with the Staff and will be combined with the O&M performance report inspection. <br /> Task 2 Permitting Forms <br /> Task 2 requires gathering the data and updating Form 1 and Form 2A as submitted for <br /> the previous permit renewal. IRC will be required to complete or provide the following: <br /> 1. Section 3. A. Discharges to Surface Water, Items 12, 13, and 14. A detailed <br /> explanation of the required information is attached in Attachment A. <br /> 2. Section 5. Industrial Pretreatment for each SIU or significant industrial users. <br /> 108 <br />
