" These files contain the description <br /> of the Nationwide Permit authorization, the Nationwide Permit general conditions , and the <br /> regional conditions , which apply specifically to this verification for NWP 13 , 7 . Enclosed is a <br /> list of the six General Conditions , which apply to all Department of the Army authorizations . <br /> You must comply with all of the special and general conditions and any project specific <br /> condition of this authorization or you may be subject to enforcement action. In the event you <br /> n ' have not completed construction of your project within the specified time limit, a separate <br /> application or re-verification may be required . <br /> The following special conditions are included with this verification , <br /> 1 . Reporting Address : All reports , documentation and correspondence required by the <br /> conditions of this permit shall be submitted to the following address : U . S . Army Corps of <br /> Engineers , Regulatory Division, Enforcement Section, P . O . Box 4970 , Jacksonville , FL 32232 . <br /> The Permittee shall reference this permit number, SAJ-2006-01903 (NW-TSD) , on all <br /> submittals . <br /> 2 . Commencement Notification : Within 10 days from the date of initiating the <br /> authorized work, the Permittee shall provide to the Corps a written notification of the date of <br /> commencement of work authorized by this permit . <br /> 3 . Completion Notification : Within 60 days of completion of the work authorized, the <br /> attached " Self-Certification Statement of Compliance " must be completed and submitted to the <br /> U . S . Army Corps of Engineers . Mail the completed form to the U . S . Army Corps of Engineers , <br /> Regulatory Division, Enforcement Section, P . O . Box 4970 , Jacksonville, FL 32232 . <br /> 4 . Cultural Resources/Historic Properties : No structure or work shall adversely affect <br /> or disturb properties listed in the National Register of Historic Places or those eligible for <br /> inclusion in the National Register. Prior to the start of work, the Applicant/Permittee or other <br /> party on the AppIicant ' s/Permittee ' s behalf, shall conduct a search of known historical properties <br /> by contracting a professional archaeologist, contacting the Florida Master Site File at 850-245 - <br /> 6440 or S iteFile6p dos . state . fl . us . The Applicant/Permittee can also research sites in the National <br /> Register Information System (NRIS) . Information can be found at <br /> hi!p : //www. cr. nT) s. gov/nr/research/. <br /> If, during the initial ground disturbing activities and construction work, there are <br /> archaeological/cultural materials unearthed (which shall include , but not be Iimited to : pottery, <br /> modified shell , flora, fauna, human remains, ceramics , stone tools or metal implements , dugout <br /> canoes or any other physical remains that could be associated with Native American cultures or <br /> early colonial or American settlement) , the permittee shall immediately stop all work in the <br /> vicinity and notify the Compliance and Review staff of the State Historic Preservation Office at <br /> 850-245 -6333 and the Corps Regulatory Project Manager to assess the significance of the <br /> 4i�17 <br /> + <br />
