To maintain at 90% the percentage of 1 . Develop and implement individual <br /> program participants who successfully meet case plans following assessment of <br /> one or more of their educational, vocational, or current level of functioning. <br /> basic living skills objectives within six months 2 . Review individual case plans every <br /> of enrollment in the program as measured by six months . <br /> documentation in their case plan . Baseline : 3 . Provide educational workshops . <br /> 2001 -2002 FY in Independent Living Program . 4 . Provide case management services . <br /> 5 . Monitor school attendance and <br /> grades . <br /> 2 .
