' t <br /> Addendum #6 <br /> 03/ 13/03 <br /> Page 2 <br /> The Stormwater Management Report shall be modified to support application to the St. Johns River Water <br /> Management District (SJRWMD) for a modification to the previously issued Environmental Resource <br /> Permit (ERP) . <br /> We shall prepare and coordinate execution of the SJRWMD application form for modification to the ERP . <br /> We shall prepare supporting information including amended plans and stormwater management reports for <br /> submittal to the SJRWMD . <br /> It is anticipated that the SJRWMD will issue a Request for Additional Information (RAI) pertaining to this <br /> permit modification request. We shall develop a response to the District ' s concerns relating <br /> to the <br /> stormwater management system design modifications . Our scope of services does not include any responses <br /> to concerns relating to wetlands and/or endangered species issues . <br /> Following approval of the ERP modification, we shall provide Indian River County with five (5) copies of <br /> the revised Construction Plan sheets and one ( 1 ) copy of the amendment to the Stormwater Report pertaining <br /> to this scope of service. <br /> The Scope of Services for this project will be expanded to provide for the above described <br /> Design/Permitting/Construction Services listed above for the Lump Sum Fee of $ 10,815 . 00 <br /> IN WITNESS WHEREOF the parties hereto have executed these presents this 1st day of Apri 1 <br /> 2003 . <br /> MASTaephen <br /> ;E. <br /> MOLER, INC .
