They may come directly from detox units, short term residential treatment <br /> stays, outpatient treatment and the streets . New Horizons of the Treasure Coast'.s <br /> professional case management staff will engage the target population in the program. The <br /> Source will assist with engagement, through sober support groups and meeting client <br /> basic needs. The Treasure Coast Homeless Services Council ' s Resource Center will <br /> support this project by providing furniture and household item and by working with <br /> professional staff to secure entitlements for this population. <br /> 3 . Discharge Planning Changes , <br /> As part of it ' s ongoing effort to participate in discharge planning which prevents <br /> discharge into homelessness, the County Commission Chair and the Director of the <br /> County Department of Human Services are part of a new Law Enforcement Partnership <br /> Committee, convened by the Treasure Coast Homeless Services Council, Inc, in <br /> February of 2003 . This Law Enforcement Partnership includes city and county law <br /> enforcement for both Indian River and St. Lucie County, two county judges and a wide <br /> variety of service providers, especially New Horizons of the Treasure Coast which <br /> provides residential mental health and substance abuse treatment to homeless <br /> individuals. The Source, which works exclusively with chronically homeless adults is <br /> also a significant member of this Committee. While no new written policies have <br /> emerged, as yet, because the applicant has no jurisdiction, several "practices " have been <br /> agreed upon among the committee participants and several new mutual understanding of <br /> the "issues " on both side have emerged. This ongoing committee is anticipated to have <br /> an impact on eliminating the revolving door syndrome chronically. homeless people <br /> experience . <br /> As in the past, Indian River County still has no direct jurisdiction over prisons, jails, <br /> hospitals or institutions but continues to work with the Department of Children and <br /> Families and FACT Teams to monitor the housing status and ongoing community <br /> maintenance of clients released from State Mental Institutions. <br /> By serving as the applicant for this project, Indian River County is also taking steps to <br /> facilitate access to permanent affordable housing for individuals who would otherwise <br /> be discharged from the county jail, local treatment programs and hospitals into <br /> homelessness . <br /> 4. Housing where participants will reside. <br /> Participants will be assisted in selecting their own housing arrangements by the <br /> Continuum of Care ' s Shelter Plus Care Committee which locates apartments in safe <br /> neighborhoods for Shelter Plus Care and other hard to place clients . Various living <br /> arrangements , including single units and group homes have been pre-identified and <br /> approved by the Committee to ensure that tenants will have access to housing that suits <br /> their needs and preferences and is also accessible to community amenities including <br /> public transportation. Accessibility to the principal service provider managing the client ' s <br /> care remains as a key factor. Apartments will be inspected by the Indian River PHA to <br /> ensure safety and compliance with all threshold requirements following the Fair <br /> Form HUD-40076-CoC(2003) <br /> OMB Approval No.2506-0112 (exp.06/30/2003) <br />
