TREASURE COAST HOMELESS SERVICES COUNCIL, INC. CONTINUUM OF CARE <br /> 1 <br /> Housing guidelines . Clients in Indian River County will also have access to the Housing <br /> Coordinator through the Homeless Resource Center to assist them with finding housing <br /> when they are ready to move from TRA to Section 8 or other permanent housing options . <br /> However, the key factor in determining if a unit or a placement is optimum will be the <br /> client ' s ability to access ongoing services and whether or not the proposed housing <br /> arrangement supports the client' s ongoing re-socialization and supportive services needs. <br /> b. Community amenities and transportation : <br /> Basic community amenities and transportation are readily available in the County, <br /> especially in the City of Vero Beach. Free public transportation is available on fixed bus <br /> routes throughout the City of Vero Beach. New Horizons of the Treasure Coast clients <br /> can also access transportation through the F. A. C . T. Team case managers. Indian River <br /> Memorial Hospital and the County Health Department, the only providers of indigent <br /> health care in the County, are located in the City of Vero Beach and are both within <br /> blocks of the Resource Center. All facilities are accessible by public transportation. <br /> c . That the apartments will be accessible to persons with disabilities. <br /> There are a number of group homes and converted assisted living facilities which have <br /> been serving as landlords for in Indian River County' s Shelter Plus Care clients when <br /> handicapped accessibility is required. There are additional units available if necessary. It <br /> is reasonable to expect that an individual with a need for handicapped accessibility will . <br /> not have difficulty locating an appropriate unit. <br /> Ideally, tenants should be located in apartments around the City of Vero Beach which <br /> provides the best accessibility to Indian River Memorial Hospital, The Health <br /> Department, numerous churches and food pantries, public transportation and the <br /> Homeless Resource Center. The Homeless Resource Center and the New Horizons FACT <br /> Team also have cars and vans available to transport clients to services and appointments. <br /> 5. Supportive services the participants will receive <br /> Supportive services will be secured and offered by New Horizon' s of the Treasure <br /> Coast 's professional treatment staff. These include ongoing medication management, <br /> mental health counseling and support and substance abuse counseling and support. In <br /> some cases, representative payees will be appointed to manage client funds . Residents <br /> will be encouraged to participate in the CoC ' s specialized intensive job rehabilitation <br /> program, which was originally designed with this population in mind. It is anticipated <br /> that most of the tenants will opt to stay actively engaged in long term supportive services <br /> like those described above due to the benefits derived from housing stability. Therefore, <br /> they will become more willing and able to work toward appropriate social and <br /> employment goals . The Shelter Plus Care Committee will monitor each tenant ' s case plan <br /> on a monthly basis to assure that case managers continue to work with each tenant to <br /> maximize their income levels through the receipt of all appropriate entitlements related to <br /> their indigence and disability. Residents will be asked to sign an agreement with their primary <br /> Form HUD-40076-CoC(2003) <br /> OMB Approval No.2506-0112 (exp.06/30/2003) <br />
