CONTINUUM OF CARE <br /> t <br /> case manager, before entering into the project, that they will stay in Indian River County and will <br /> comply with their treatment plan. <br /> Specialized training for this population's disability needs will include : <br /> Medications Management <br /> Mental Health Counseling and Ongoing Case Management <br /> Drop-In Center Re-Socialization <br /> Participation in Self-Help Recovery Support Groups <br /> Consumer Skills <br /> Nutrition Education <br /> 6. Self-Sufficiency <br /> A comprehensive plan will be created for each client by . the primary case manager. A <br /> new case record will be opened and maintained for each client using the Continuum ' s <br /> new FMS system, Service Point. It will track all efforts to determine and secure <br /> entitlements for which the clients are eligible. At a minimum, ongoing medication and <br /> treatment requirements and potential for increased income through employment and <br /> mainstream entitlement will be addressed. Entering this information will be the <br /> responsibility of the particular case manager who is responsible for assisting the client <br /> through the process . Every client in the Shelter Plus Care Program is automatically <br /> personally assisted with securing Medicaid, SSI, SSDI, Vet Benefits, Food Stamps and <br /> Supplementary Medication Assistance by the primary case manager who recommends the <br /> client to the program. The Resource Center provides additional assistance to the clients <br /> until the process is complete. The process includes( 1 )automatically applying for Section <br /> 8 on behalf of each client, (2) completing simultaneous paperwork with the client for <br /> various entitlements all at once, (3 ) making the appointments, (4) following up on <br /> appointments, (5) securing transportation if necessary, (6) selecting representative <br /> payees, when necessary, (6) advocating on behalf of the client with social security and <br /> disability determination staff and (7) securing free legal advocacy for clients who are <br /> denied, when necessary. The responsible service provider(s) will be identified in the <br /> plan, as well as other agencies who will participate in service delivery. A monthly report <br /> of services received by each client is provided to the Shelter Plus Care Committee which <br /> monitors service activity and the level of increase in income and entitlements for each <br /> client. Where feasible, employment training and job finding assistance will also be <br /> provided by the Homeless Resource Center to these clients to support a goal of at least <br /> 40 % of these clients moving into Section 8 or their own housing during the five year <br /> grant period. <br /> Foam HUD-40076-CoC(2003) <br /> OMB Approval No.2506-0112 (exp.06/30/2003) <br />
