ADDENDUM 413 TO CONTRACT DATED JANUARY 21 , 1997 BETWEEN INDIAN <br /> RIVER COUNTY AND MASTELLER & MOLER, INC. FOR CR 512 PAVING & <br /> DRAINAGE IMPROVEMENTS <br /> INDIAN RIVER COUNTY PROJECT #9611 <br /> COUNTY ROUTE 512 CORRIDOR IMPROVEMENTS <br /> ROSELAND ROAD TO I-95 <br /> INDIAN RIVER COUNTY, FL <br /> As result of discussion with and comments from Indian River County Staff during the <br /> finalization of the construction plans for Phase IV of the CR-512 Roadway Improvements, it has <br /> been noted that work not included in the original contract or subsequent addendums must be <br /> performed in order to ensure a successful project. The anticipated additional work shall include <br /> turn lanes, the redesign of the 101st Avenue/CR-512 intersection, the evaluation of existing <br /> drainage structures on the Sebastian Middle School site, an updated environmental survey of the <br /> Phase IV alignment, the design and permitting of a sidewalk extension along the north side of <br /> CR-512, and the design of a north-bound left tum lane on 101st Avenue at CR-512. <br /> In order that the project design may be modified and permits obtained, we will perform the <br /> following "Additional Services": <br /> Task A — Phase IV Construction Plan Modifications <br /> We will add the following design elements to the construction plans as requested by the County: <br /> 1 . Additional Rip_ht-Turn Lanes: In accordance with the County' s request, we will design right <br /> turn lanes at the following locations: <br /> a. 101st Avenue (east-bound) <br /> b. 104th Avenue (west-bound) <br /> c. 106th Avenue (west-bound) <br /> d. 108th Avenue (west-bound & east-bound) <br /> 2. Redesign of 101st Avenue Intersection: The County has proposed to realign 102"d Avenue <br /> north of CR-512 to meet the intersection of 101 " Avenue and CR-512. As such, the Phase <br /> IV construction plans will be revised to illustrate a plateau intersection at that location with a <br /> tumout to the north for the future connection of 102"d Avenue. <br /> 3 . Traffic Signal Interconnection: In order to extend traffic signal interconnect capability along <br /> the Phase IV portion of CR-512 we will incorporate the design of double 2-inch conduit with <br /> related pull boxes into the Phase IV construction plans. The design of the traffic interconnect <br /> conduit will be coordinated with the County' s Traffic Engineering department. <br />
