t <br /> IMPROVEMENT OF 43RD AVENUE L'9) 7- 2 <br /> PROFESSIONAL CIVIL ENGINEERING & STRUCTURAL ENGINEERING <br /> SERVICES <br /> AMENDMENT NO. 1 TO WORK ORDER NO. 1 TO THE PROFESSIONAL <br /> ENGINEERING/LAND SURVEYING SERVICES AGREEMENT BETWEEN ARCADIS <br /> AND INDIAN RIVER COUNTY, FLORIDA, <br /> This is an amendment to the existing Engineering Services Agreement (AGREEMENT) dated <br /> September 6, 2005 between ARCADIS (ENGINEER) and Indian River County (COUNTY). <br /> This amendment addresses changes in " Section I — Project Limits and Description", "Section III - <br /> Scope of Services", and " Section V - Compensation" . <br /> Amendment Description <br /> This Amendment includes the following: <br /> "SECTION I - PROJECT LIMITS AND DESCRIPTION" is being modified to incorporate <br /> the following: <br /> Consultant services are required for the preparation of road design plans; bridge plans; <br /> permit applications and utility relocations associated with the construction of 43rd Avenue <br /> Improvements. The improvements consist of the following: <br /> Improvement of the intersection of 43`d Avenue & 5th Street SW to include a single left <br /> turn lane and two through lanes on each approach. ARCADIS is currently preparing plans <br /> for the widening of 43` Avenue from Oslo Road to 8th Street as a 4 lane section. <br /> ARCADIS will prepare a separate set of "interim" plans consisting of the replacement of <br /> the bridge at 5th Street SW, the installation of a mast arm traffic signal at the intersection <br /> of 43 `d Avenue and 5th Street SW, and the installation of the left tum lanes. Additional <br /> widening will be provided on the north approach to accommodate u-tuming vehicles. <br /> 5th Street SW Phase 1 <br /> Limits of project along 43rd Avenue will extend from approximately 7501f south <br /> of 5th Street SW to approximately 560 if north of 5d' Street SW, and along 5ch <br /> Street SW from approximately 500 feet west of 43`d Avenue to approximately 800 <br /> feet east of 43`d Avenue for a distance of 2,610 if or 0.49 miles. <br /> 1st Street SW Left Turn Lanes <br /> During the 30% plan review, Indian River County requested that left turn lanes be <br /> designed on 1st Street (Carter Road) . Additional field survey and engineering <br /> design work is required to incorporate the left turn lanes into the plans. The field <br /> survey will consist of a topographic field location survey from approximately 800 <br /> feet west of 43 `d Avenue to 600 feet east of 43`d Avenue. A survey baseline and <br /> benchmarks will be set in the field.
