Amendment No. 3 to <br /> 43rd Avenue Roadway and Drainage Improvements <br /> Professional Engineering Services Agreement <br /> of the project. Site visits are in addition to the job progress meetings and will <br /> be made at an average frequency of twice per week (80 site visits total based <br /> on l8`h months of active construction) . <br /> 5 . Submittals : Review samples, catalog data, schedules, shop drawings, <br /> laboratory, shop and mill tests of materials and equipment and other data the <br /> contractor submits in accordance with the Contract Documents. <br /> 6 . Payment Applications : Review the Contractor' s applications for payment <br /> and make a recommendation to the COUNTY for payment. <br /> 7 . Interpretations : Furnish consultation regarding interpretations of the project <br /> plans, technical specifications, and permit conditions. <br /> 8 . Changes : Review proposal requests, supplemental drawings and <br /> information, and change order requests. <br /> 9. Utility Coordination : Coordinate with utilities regarding conflict resolution, <br /> relocation, and/or abandonment of existing facilities. <br /> 10. Punch List: Conduct final inspection of the project for compliance with the <br /> Contract Documents, prepare final "punch list" of deficiencies, and conduct a <br /> verification inspection to ensure that deficiencies are corrected. <br /> H. Final Certifications <br /> 1 . As-Built Survey : The ENGINEER will conduct an "as-built" survey of the <br /> constructed improvements, including inverts of storm sewer piping at <br /> structures. The survey shall be the minimum required for final certifications . <br /> 2 . Record Drawings : The ENGINEER will prepare "record" drawings based <br /> on the "as-built" survey, "marked-up" drawings and other information <br /> provided by the Contractor and the COUNTY. <br /> 3 . Final Certifications : The Engineer will provide final certifications to the <br /> Indian River Farms Water Control District and the St. Johns River Water <br /> Management District. <br /> SECTION IV - TIME FOR COMPLETION <br /> The time for completion of the preliminary (60% design) plans shall be twenty (20) weeks. <br /> After COUNTY review, the time for completion of the final ( 100% design) plans, shall be twenty <br /> (20) weeks. After COUNTY review, the time for completion of the permit applications shall be six <br /> (6) weeks. After receipt of permits, the time for completion of the contract documents (plans and <br /> specifications) shall be six (6) weeks . <br /> Page 5 of 8 <br />
