11 .4 . Bids by partnerships must be executed in the partnership name and signed by a <br /> partner, whose title must appear under the signature and the official address of the <br /> partnership must be shown below the signature. <br /> 11 . 5 . All names must be typed or printed below the signature. <br /> 11 . 6 . The Bid shall contain an acknowledgment of receipt of all Addenda (the numbers <br /> of which shall be filled in on the Bid Form) . <br /> 11 . 7 . The address to which communications regarding the Bid are to be directed must <br /> be shown. <br /> 11 . 8 . All supporting information requested in the Bid Form must be furnished. Do not <br /> leave any questions or requests unanswered. <br /> 12 . Submission of Bids <br /> 12 . 1 Bids shall be submitted at the time and place indicated in the Invitation to Bid and <br /> shall be included in an opaque sealed envelope, marked with the Project title and <br /> name and address of the BIDDER and accompanied by a completed Section <br /> 00411 , Statement on Public Entity Crimes , a completed Section 00412 , Sworn <br /> Statement Under the Florida Trench Safety Act, sworn statement under Section <br /> 105 . 08 , Indian River County Codes, the Bid Security and other required <br /> documents which are included in the Contract Documents . If the Bid is sent <br /> through the mail or other delivery system the sealed envelope shall be enclosed in <br /> a separate envelope with the notation "BID ENCLOSED " on the face thereof. <br /> 12 . 2 The successful BIDDER will be responsible for compliance with all applicable <br /> safety-related Federal and State statutes and regulations, including, but not limited <br /> to , the OSHA excavation safety standards, 20 C .F .R. , Part 1926 , Subpart P- <br /> Excavations , Trenching and Shoring, which are adopted by reference of the <br /> Florida "Trench Safety Act" , Section 553 . 60 et. seq. , Florida Statutes . The <br /> BIDDER who will perform trench excavation work on a construction project with <br /> trench excavation(s) over 5 feet in depth must allocate and include in its bid the <br /> cost of compliance with trench safety standards and any special shoring <br /> requirements . The BIDDER must indicate in the sworn statement the method(s) <br /> he intends to use to achieve compliance with each item (i . e . , trench box , sheet <br /> piling, cribbing, wood shoring, or other method selected by BIDDER) . <br /> 13 . Disqualification of BIDDERS <br /> 13 . 1 One Proposal : Only one Proposal from an individual firm, partnership or <br /> corporation under the same or under different names will be considered . If it is <br /> believed that a BIDDER is interested in more than one proposal for the work <br /> involved, all Proposals in which such a BIDDER is interested will be rejected . <br /> INSTRUCTION TO BIDDERS <br /> IB - 7 <br />
