CHANGE ORDER 2, 091- Nil <br /> No. 1 <br /> PROJECT West Wabasso Water Lines <br /> DATE OF ISSUANCE EFFECTIVE DATE <br /> OWNER Indian River County <br /> OWNER ' s Contract No. Project No. U ED BY <br /> CONTRACTOR : Sheltra and Son Construction Inc. <br /> ENGINEER Indian River County/Himanshu Mehta— FEB 1 3 2007 <br /> You are directed to make the following changes in the Contract Documents : BOARD OF COUNTY <br /> Cn � urrni�N <br /> Item # 1 Description : <br /> Deduct 530 LF of 6" PVC water main at a unit price of $22 .00 (Total $ 11 , 660. 00) and Add <br /> 530 LF of 6" HDPE water main at a unit price of $ 75 . 00. ($ 39,750 . 00) <br /> Reason for change order: Total $ 28,090.00 <br /> a) Approximately 280 LF of directional bore (HDPE) was needed on 87th Place. In general , the <br /> Contract drawings specified a standard open & cut method for installation of the 6 " PVC water <br /> main along the property line. The primary reason to employ the directional bore method was to <br /> avoid obstructions such as tall dense trees and vegetation area, to avoid a fence and a garden, and <br /> to avoid compromising a residential trailer that was too close to the property line. These <br /> obstructions were not identified on the survey that was used for the design. <br /> b) Approximately 180 LF of directional bore was needed at the end of 61 st Drive. Although <br /> most of the road is paved, the last 210 LF of the road was heavily vegetated. Per the Contract <br /> drawings, this area was to be cleared along the path of the water main. Upon clearing most of <br /> this area, it was determined that first there was a significant drop in elevation and second that <br /> there was water pooled at the end of the road. The directional bore method was selected to avoid <br /> extensive dewatering and handling of the water and to avoid backfilling the area with soil . Both <br /> of these items were not in the scope of the project. These were unknown conditions at the time of <br /> the design since the survey did not identify these possible conditions . <br /> c) Approximately 70 LF of directional bore was needed on 61 st Drive to avoid open & cut <br /> installation of a new residential concrete driveway that was not identified on the survey at the <br /> time of the design. <br /> Item #2 Description : <br /> Add additional 120 LF of 4 " PVC water main at the unit price of $ 19 .00 ( S2,280. 00).- <br /> Total <br /> 2280 00).Total $ 2,280.00 <br /> Reason for change order: The Contract drawings specified extending 160 LF of 4 " PVC to <br /> provide water services for seven homes into an area that was not properly platted . This extension <br /> was designed to minimize the long service lines that were needed to reach these homes. <br /> However, upon installation of this water main it was determined that the location at which this <br /> pipe ended was not in a suitable location and that it did not adequately serve the purpose for <br /> minimizing the long service line. Therefore, an additional 120 LF of 4" PVC water main was <br /> needed in which the shorter service lines provide a better water pressure to the house. <br />
