How will the program be accessible to target population (i. e. , location , transportation , <br /> hours of operation) ? <br /> The Independent Living Program ensures that the program is accessible to the target <br /> population by providing services in the client' s home and by providing transportation to <br /> activities and workshops as needed . Monthly case management visits occur in the client' s <br /> home. Workshops are scheduled in the clients ' county of residence and transportation is <br /> offered to ensure clients can attend. Visits and events are often scheduled in the evenings <br /> and on weekends when the youth are not in school in order to make these services easily <br /> accessible. <br /> 9 <br /> t <br />
