CHILDREN ' S HOME SOCIETY INDEPENDENT LIVING PROGRAM <br /> Indian River County Children ' s Services Advisory Committee <br /> G. TIMETABLE (Section G not to exceed one page) <br /> 1 . List the major action steps , activities , or cycles of events that will occur within the <br /> program year. New programs should include any start-up planning that may occur <br /> outside the funding year. In completing the timetable , review information detailed in <br /> prior sections . <br /> Month/Period Activities <br /> 10/04 to 9/05 Monthly educational/vocational workshops wi ; ; be presented to <br /> program participants . <br /> Case management services will be provided to all program participants <br /> Educational presentations regarding the Independent Living Program <br /> will be presented to DCF, CHS , and community agencies . <br /> Program staff will attend staffings at the DCF service center to identify <br /> eligible youth for referral to Independent Living . <br /> Program staff will attend quarterly statewide Independent Living <br /> Coordinators meetings . <br /> Within 30 days of <br /> referral Program staff will complete an assessment and develop an individual <br /> case plan for each program participant . <br /> This program is staffed and fully operational at the present time ; <br /> therefore, a start up timetable is not necessary . <br /> 14 <br />
