TIMETABLE (Section G not to exceed one page) <br /> IL List the major action steps, activities, or cycles of events that will occur within the <br /> program year. New programs should include any start-up planning that may occur <br /> outside the funding year. in completing the timetable, review information detailed in <br /> prior sections. <br /> Month/Period Activities <br /> ongoing • Families are referred by United for Families and their collaborative <br /> partners_ <br /> • Families are assessed and safety plans are developed with the <br /> family to meet their identified needs. <br /> • Services and interventions are provided as identified on plan. <br /> • Cases are closed as needs are addressed and risk level to family and <br /> child is diminished. <br /> • Cases are staffed with collaborative partners to identify new <br /> Weekly families and update existing cases. <br /> • Cases are staffed with supervisor to ensure appropriate level of <br /> intervention to meet the needs of the family. <br /> • Risk Management Committee reviews any significant events that <br /> Monthly occurred ding the month involving the program and/or <br /> individuals served. <br /> • Reports are submitted on cumulative client demographics. <br /> Quarterly Records are reviewed as part of internal Peer Record Review to <br /> ensure compliance with funder, Council On Accreditation, and <br /> "best practice" standards. <br /> Six Months Narrative report submitted on progress toward program outcomes. <br /> Annualiy • End of year narrative report given on progress toward program <br /> outcomes. <br /> • Monitoring occurs by United for Families to determine if any <br /> verified incidents of abuse or neglect occurred during service <br /> provision or within one year of completion of services. <br /> • Monitoring occurs by Children' s Services Councils and United <br /> Ways for program effectiveness and efficiency. <br /> 11 <br />
