GENERAL INFORMATION <br /> ■ <br /> The original and three (3) copies of your bid shall be submitted to Procurement Division at the <br /> time and date specified . At the designated time and place, the City Procurement Official or designee <br /> ■ <br /> will publicly open the bids . These documents constitute the complete set of specification <br /> requirements and bid forms. It is the responsibility of the bidderto insure thatall pages are included . <br /> Therefore, all bidders are advised to closely examine this package . Any questions regarding the ■ <br /> completeness of this package should be immediately directed to the Procurement Division . ALL BID <br /> PROPOSALS MUST BE SUBMITTED ON THE PROVIDED INVITATION FOR BID " PROPOSALS" ■ <br /> FORM . BID PROPOSALS ON VENDOR LETTERHEAD/QUOTATION FORMS WILL NOT BE <br /> ACCEPTED . w <br /> All bid proposals must be typed or written in ink, and must be signed in ink by an officer or employee ■ <br /> having authority to bind the company or firm . SIGNATURES ARE REQUIRED WHERE <br /> INDICATED , FAILURE TO DO SO SHALL BE CAUSE FOR REJECTION OF BID. ■ <br /> Bidders shall not be allowed to modify their bids after the opening time and date.
