i <br />O� f J <br />WORK ORDER #4 <br />ENVIRONMENTAL ASSESSMENT AND REGULATORY PERMITTING <br />CR 510 / ROAD WIDENING <br />EAST SECTION (EAST OF 66TH AVENUE) TO INDIAN RIVER LAGOON <br />This work order #4 is for the continuing contract agreement with G. K. Environmental, Inc. for professional <br />services entered into effective as of August 12, 2008 by and between INDIAN RIVER COUNTY, a political <br />subdivision of the State of Florida, 1801 27'h Street, Building A, Vero Beach, Florida 32960 ("County") and G. K. <br />ENVIRONMENTAL, INC., 155 McKee Lane, Vero Beach, Florida 32960, ("Consultant"), <br />SECTION I = PROJECT DESCRIPTION as follows: Indian River County will be widening CR 510 <br />beginning at the intersection of CR 512 and CR 510 and continue to east to the Indian River Lagoon. This <br />work order will be to complete environmental assessments and regulatory permitting for the eastern section. <br />The following environmental services are required: <br />1.0 WETLAND JURISDICTIONAL SERVICES AND ENDANGERED & THREATENED / <br />SPECIES OF SPECIAL CONCERN SURVEYS <br />2.0 PERMITTING SUBMITTAL ASSISTANCE <br />3.0 MITIGATION DESIGN AND OPTIONAL SERVICES <br />4.0 RESPONSE TO AGENCY REQUESTS FOR ADDITIONAL INFORMATION (RAI) <br />SECTION II — DETAILED SCOPE OF SERVICES: <br />The preparation of documents, compiling of data, and meeting attendance resulting from field <br />investigations required for environmental permitting of this project. <br />The ENVIRONMENTAL CONSULTANT agrees to perform professional environmental consulting <br />services in connection with the project as required and set forth in the following: <br />DETAILED SCOPE OF SERVICES <br />PHASE I <br />1.0 WETLAND JURISDICTIONAL SERVICES AND ENDANGERED & THREATENED / SPECIES OF <br />SPECIAL CONCERN SURVEYS <br />1.1 Collection of Existing Data <br />GKE will collect and review soils, topography, and National Wetland Inventory data in preparation for the <br />field data collection effort. <br />1.2 Field Determination of Approximate Wetland Lines <br />A field review will be conducted to establish approximate wetland boundaries. GKE biologists will flag / stake <br />the approximate wetland boundaries in accordance with federal and state criteria (Federal Manual for <br />Identifying and Delineating Jurisdictional Wetlands. 1987 and Chapter 62-340 of the Florida Administrative <br />Code).
