<br /> F. PROGRAM EVALUATION (Entire Section F not to exceed two pages)
<br /> 1 . DEMOGRAPHICS : What information (data elements) will you need to collect in order
<br /> to accurately describe your target population including demographics (age, gender and
<br /> ethnic background) required by the funder in Section H? What are the pieces of
<br /> information that qualify them for your target population? How do you document their
<br /> need for services or their "unacceptable condition requiring change" from Section Bl ?
<br /> The information that will need to be collected include age, earnings for 6-month period prior to
<br /> application, pregnant and/or parenting, education level, basic skills level, employment status,
<br /> ages of children, and child care status .
<br /> Information that qualifies applicants for the target group include eligibility determination for
<br /> OSY or Front Porch Family Literacy Program, which consists of age, income, education level,
<br /> basic skills level, pregnant or parenting status, employment status, and street address.
<br /> The need for services will be documented through a needs assessment. Once parenting youth
<br /> have been identified, they will meet with the Client Services Coordinator, who will discuss with
<br /> the parent their child care status. If children are not enrolled in a child care program, the parent
<br /> will be required to advise of the reasons why they are not enrolled. If they are on a waiting list,
<br /> verification must be obtained; if they have not applied, a referral will be made to ALPI and
<br /> CCCR, and depending upon the ages of the children, EOC Head Start Program. Once
<br /> application has been made, verification of approval, denial/reason for denial , or
<br /> placement/location/estimate time on the waiting list must be obtained.
<br /> 2. MEASURES : What data elements will you need to collect to show that you have
<br /> achieved (or made progress toward) your Measurable Outcomes in Section D? What
<br /> tools or items are you using as measures (grades, survey scores, attendance, absences,
<br /> skill levels) for your program? Are you getting baseline information from a source on
<br /> your Collaboration List in Section E? Are there results from your Activities in Section
<br /> D that need to be documented ? How often do you need to collect or follow-up on this
<br /> data?
<br /> In order to show progress toward measurable outcomes, information that will be collected
<br /> includes eligibility documentation, child care enrollment forms, certificates of completion,
<br /> employment information, and follow up information such as dates of employment and earnings.
<br /> Tools that will be used to measure outcomes include Program Application, ALPI and CCCR
<br /> Waiting Lists, Needs Assessment, WIA Self-Certification Form, and Program/ Employment
<br /> application. Some baseline information will be provided by collaborative partners, while other
<br /> information will be provided by the parent or determined during the application, intake, and
<br /> assessment processes. Data will be collected on an on-going basis and reported quarterly to
<br /> document activities and outcomes. Follow up will be conducted for one year after exit, at 30
<br /> day, 60 day, 90 day, 6 month, 9 month, and 12 month intervals . Follow up information will be
<br /> obtained through employment verification, worksite visits, and counseling/job coaching.
