In <br /> addition, the report must be accompanied by a Ground Water Monitoring Repoil form. <br /> Task 5 — CVOC Plume Monitoring Reporting <br /> The results of groundwater sampling associated with the CVOC plume monitoring <br /> will be presented in a report supported by attachments containing Chain of Cusstody sheets , <br /> field notes and observations , water sampling logs , maps, graphs, analytical results , and any <br /> other applicable materials . This report will be prepared under Task 5 and will lie submitted <br /> to IRC for review prior to submittal to FDEP . The semi-annual CVOC Plume Monitoring <br /> Reports will be submitted to FDEP within sixty (60) days of receipt of the analytical data <br /> from the laboratory. In addition, the well installation activities will be documented in the <br /> initial semi-annual report, supported by attachments containing well completion forms , <br /> field notes, and observations . <br /> Task 6 — Quarterly Remedial System Reporting <br /> As outlined in the July 2003 RAP, letter reports will be prepared and submitted to <br /> FDEP on a quarterly basis . The reports will document : (i) laboratory analytical results for <br /> collected influent and effluent groundwater samples ; (ii) volume of groundwater extracted; <br /> (iii) graphs of influent and effluent concentrations versus time ; and (iv) any specific <br /> maintenance activities or system upsets . The budget for two years (eight report ng periods) <br /> of quarterly reporting has been included under this task . However, following t e first year <br /> of quarterly letter report submittals, the frequency of submittals will be re-evaluated and <br /> potentially reduced to semi-annually (with prior FDEP approval) . Sincesemi-annual <br /> groundwater monitoring reports for the CVOC plume monitoring network are prepared for <br /> the site (which include groundwater level measurements and monitoring wia.11 network <br /> sampling results) , the treatment system letter report submittals are intended to simply <br /> provide operational data associated with the expanded P&T system. <br /> BUDGET <br /> The budget estimate for the proposed activities described herein is f rovided as <br /> Attachment B .
