Mr. Terry Southard <br /> 16 December 2008 <br /> Page 5 <br /> Task 3 — Site Assessment Report Preparation <br /> The FDEP requires two copies of the SAR be submitted to the FDEP . Please note the <br /> assessment efforts described above may or may not be sufficient to collect the data needed to <br /> satisfy SAR requirements ( in particular, we do not at this time know if the above efforts will be <br /> sufficient to delineate the extent of contamination) . If we find during completion of the above <br /> tasks that additional site assessment work is necessary , we will discuss our recommendations <br /> with you and seek approval prior to proceeding If additional tasks are required beyond the <br /> scope of this proposal additional budget will be required . <br /> The SAR will describe the tasks that were completed pursuant to Chapter 62 - 780 FAC and will <br /> summarize the obtained results . The SAR will include recommendations for additional <br /> monitoring and/or remediation . <br /> In an effort to address FDEP guidance, the following will be included in the SAR : <br /> • a brief summary of the site history and operations ; <br /> • a copy of the portion of the USGS topographic map that identifies the site in relation to <br /> the surrounding area ; <br /> • a vicinity map showing pertinent site features, such as utilities, aboveground and <br /> underground structures, storage areas, local drainage features, land cover , facility <br /> boundaries, and potential sources of contamination identified during the assessment ; <br /> • tables listing the monitoring well top-of-casing elevations, depths to groundwater and <br /> water- level elevations (obtained at least twice , at least one month apart and the dates the <br /> data were collected) ; <br /> • tables summarizing the groundwater analytical results, reporting limits used , and <br /> analyses performed ( listing contaminants detected and the corresponding cleanup target <br /> levels proposed for use for the site) ; <br /> • a description of the site-specific stratigraphy, based on the lithologic logs prepared <br /> during soil boring and monitoring well installation ; <br /> • details of assessment methodologies used at the site, including field screening techniques <br /> and other testing ; <br /> • monitoring well construction details and construction diagrams , including methods and <br /> materials , field sampling data sheets, lithologic logs , and volumes of groundwater <br /> removed during well development and/or purging ; <br /> FR1329/XR08264 , DOC <br />
