DEMOGRAPHICS : What information (data elements) will you need to collect in order to <br /> accurately describe your target population including demographics (age, gender, and ethnic <br /> background) required by the funder in Section H? What are the pieces of information that <br /> qualify them for your target population ? How do you document their need for services or <br /> their "unacceptable condition requiring change" from Section B19 <br /> Data Elements Needed to Describe Target Population <br /> 1 . Families with children 0- 5 years old <br /> 2 . Residence in Indian River County <br /> 3 . Income no greater than 200% of the Federal Poverty Guideline <br /> 4 . Employment and/or school no less than 30 hours per week <br /> 5 . Participation in required parent programs <br /> 6 . Payment of tuition on a sliding fee scale <br /> QualiAing Documentation: <br /> 1 . Driver' s License <br /> 2 . Social Security for parents and child, or green card for non-citizens <br /> 3 . Immunization Record <br /> 8 . Birth Certificate of Enrolling Child <br /> CCCR uses QuickBooks Pro and ProCare software programs to document and track the above . Each <br /> child has a folder with application, copies of qualifying documentation, and parent <br /> contract/permissions . Employment and income status is spot checked throughout the year. <br /> 2 . MEASURES : What data elements will you need to collect to show that you have achieved <br /> (or made progress toward) your Measurable Outcomes in Section D ? What tools or items <br /> are you using as measures (grades, survey scores, attendance, absences , skill levels) for your <br /> program ? Are you getting baseline information from a source on your Collaboration List in <br /> Section E ? Are there results from your Activities in Section D that need to be documented ? <br /> How often do you need to collect or follow-up on this data ? <br /> Measurement Elements: Goal I (To acquire school readiness skills) <br /> a. Ages and Stages developmental screening tool for infant' s and toddlers, administered fall and <br /> spring . <br /> b . Speed Dial R developmental assessment for preschoolers , administered fall and spring . <br /> c . DECA assessment for emotional and social , ages 2-4 years, administered fall , spring and as <br /> needed <br /> d . Kindergarten Transition Form, completed spring prior to kindergarten <br /> e . Tracking CCCR children through the school system, fall of kindergarten <br /> Measurement Elements : Goal 2 (Program Quality) <br /> a. Standards of Quality Report, submitted quarterly by CCCR directors documenting staff turnover, <br /> teacher salaries , student : teacher ratios , and staff in-service hours <br /> b . Copies of center accreditations, licensing, Director' s credentials , Gold Seal , and liability <br /> insurance kept on file in the CCCR office <br /> 11 <br />
