Community Child Care Resources, Inc. Psychological Services Children 's Services Advisory Committee <br /> 3 . To increase school readiness through a 10% 1 . Classroom teacher completes a request for <br /> increase in appropriate behavior in classrooms psychological services (Observation report), and <br /> receiving intervention as measured by initial documents specific inappropriate behaviors <br /> classroom teacher report and therapist observation requiring intervention. <br /> report 2 . The contracting CCCR therapist schedules two- <br /> hour time blocks . <br /> Baseline : Classroom observation report for 3 . During the first visit, the Psychological Services <br /> psychological services and Therapist Classroom Classroom Report is completed. <br /> Services Report 4 . The therapist works with the teacher to enhance <br /> the classroom environment, and supports the <br /> teacher with behavioral concerns . <br /> 6 . The therapist helps the teacher develop a behavior <br /> management plan, and conferences with the Center <br /> Director regarding implementation of the plan. <br /> 7. The CCCR Family Resource Coordinator (FRC) <br /> follows-up on a regular basis with the classroom <br /> teacher to help implement, adjust and evaluate the <br /> plan . <br /> 8 . If needed, the therapist makes additional visits <br /> and/or suggestions for individual child referrals . <br /> Application for 2005 -2006 service period 9 <br />
