Hibiscus Childrens Center <br /> Crisis Nursery Program <br /> Indian River Children 's Services Advisory Committee <br /> plan is developed. Follow-up takes place by comparing scores in the next review cycle to <br /> demonstrate improvement. <br /> Clients are informed in writing of our Continuous Quality Improvement processes and are <br /> allowed access to reports generated upon request. These reports are also available to funders <br /> upon request. Additionally, Hibiscus maintains an adequate MIS to comply with all funders <br /> requests for documenting and sharing data elements . <br /> Change in the family ' s situation is most clearly documented through the completion of the <br /> Family Support Plan. This documentation is maintained in the individual family ' s record. The <br /> systemic change of preventing and reducing child abuse is documented through our annual <br /> monitoring by DC&F . <br /> The Department of Children & Families will report outcomes on abuse and/or neglect that <br /> occurs during services . Prior to service initiation, families are screened through the Department <br /> of Children & Families to determine if there is an open case of abuse or neglect. This <br /> information is verified by the Department and maintained in client records . At all times during <br /> services, from initiation to completion, Family Advocates are working with the families they <br /> serve to collect accurate data. <br /> Data is collected and compiled on a monthly basis by the Family Advocates and presented to <br /> the Program Director. The Program Director reviews all program data for accuracy and <br /> reporting. It is compiled by county and rolled up into a report that is provided to all funding <br /> entities based on their requirements individually and in full to the Department of Children & <br /> Families . This data is included in the agency ' s yearly annual report, is utilized in brochures and <br /> other agency materials, and is available upon request. <br /> 11 <br />
