COLLABORATION (Entire Section E not to exceed one page) <br /> 1 . List your program' s collaborative partners and the resources they are providing to the <br /> program beyond referrals and support. (See individual funder requirements for <br /> inclusion of collaborative agreement letters. <br /> Collaborative Agency Resources provided to the ro ram <br /> The C.A.S.T.L. E . * Provides administrative and fiscal oversight for <br /> (Contracted service agency for contracted HF-IRC service staff. <br /> HF-IRC services) * Assist the IRCHSC in marketing and increasing <br /> awareness of HF-IRC in Indian River County. <br /> * Provides in-house programs for referral to HF-IRC <br /> and from HF-IRC families . <br /> All these services are provided in-kind, with no <br /> administrative fee from BOCC-CSAC. <br /> Indian River County Health * Provides coordination of HS/HF prenatal screen. <br /> Department * Communication on potential HF client ' s status . <br /> * Medical/nursing based services for HF client on an <br /> "as-needed" basis . <br /> * WIC and needed health care services at CHD clinics . <br /> All these services are provided in-kind <br /> Indian River Memorial Hospital * Provides referrals to HF-IRC program from Delivery/ <br /> Nursery Department for identified high-risk families . <br /> All these services are provided in-kind. <br /> IRC Library — Born to Read * Provides two books to newborn families that are given <br /> Program to the family of the newborn by the Stork Club . The <br /> HF-IRC emphasizes the importance of reading to their <br /> infant towards enhancing brain development. <br /> All these services are provided in-kind. <br /> Visiting Nurses Association of the * Provides Home visitation for mothers of newborns <br /> Treasure Coast experiencing breastfeeding difficulties or in need of <br /> greater breastfeeding education. <br /> Partners in Women 's Health * Primary site for Healthy Families referrals .
