Indian River County Healthy Start Coalition, Inc. — 2003-04 TLC Newborn Program <br /> IRC Board of County Commissioners — Children 's Services Advisory Committee <br /> 42% for the Healthy Start Infant/Postnatal Screen. This indicates nearly half of the pregnant <br /> women or infants in our county are NOT being screened for poor risk factors and could <br /> potentially be missed in terms of needing education and support. This is acceptable from a <br /> service delivery perspective, due to the limited staff of Healthy Start Care Coordination not <br /> being able to serve more than a few hundred families for ongoing case management anyway. <br /> But TLC fills this gap in care and services and provides a safety net for any families that were <br /> not screened for Healthy Start and might need additional case management or nursing services <br /> that Healthy Start or Healthy Families can offer. One of the best indicators of the success of the <br /> TLC program, in addition to the Healthy Start and Healthy Families programs , is the very low <br /> infant mortality rate in the last two years, which has been at 3 . 6 and 3 . 9 infant deaths per 1 , 000 <br /> live births in Indian River County, with postnatal infant deaths (infant dying between 28 to 365 <br /> days of life) being extremely low and way below the state average, which is the category that <br /> TLC services would most likely prevent. <br /> 4. List staffing needed for your program, including required experience and estimated <br /> hours per week in program for each staff member and/or volunteers (This section should <br /> conform with the information in the Position Listing on the Budget Narrative Worksheet). <br /> TLC Coordinator — 30 hours per week (75 % Full Time Equivalent - FTE) <br /> TLC Assistant Coordinator — 20 hours per week <br /> Three (3) TLC Family Associates - 10 hours per week each (one Family Assoc . is bi-lingual) <br /> Secretary — 10 hours per week <br /> The TLC staff have nursing, psychology or child education backgrounds and all have college <br /> degrees . The continuity and professionalism of the current staff have been one of the <br /> cornerstones for theprogram ' s success over the last five years . <br /> 5. How will the target population be made aware of the program? Contact is made at the <br /> hospital with the mother and family of the newborn. TLC Newborn brochures, which describe <br /> the services, are at Indian River Memorial Hospital, Indian River County Health Department, <br /> and obstetric medical providers ' offices and distributed at health fairs . Hospital personnel <br /> enthusiastically describe and endorse TLC Newborn to the mothers . Because the program is <br /> universal in nature, much of the awareness comes from word of mouth from the 1 ,000 new <br /> moms and families the program serves each year, as well as from the grandparents and relatives <br /> of the newborn . The IRCHS Coalition also markets the program through its newsletter, public <br /> presentations, every other month Coalition meetings, and at health fairs or other public events . <br /> 6. How will the program be accessible to target population (i. e. location, transportation, <br /> hours of operation) ? Visits to Indian River Memorial Hospital are made six out of seven days <br /> of each week by the TLC Newborn personnel who meet the mother of every newborn either the <br /> day of delivery of her baby or the day after. Infants born on Sunday, are normally contacted on <br /> the second day after delivery. Follow up phone calls are made with each family of a newborn <br /> who is interested in participating in the TLC Newborn program, which is over 95 percent of all <br /> newborn families seen by TLC in Indian River County, <br /> 7 <br />
