i. <br /> �to69 - 310 <br /> INDIAN RIVER COUNTY <br /> Civil Engineering and Land Surveying for Intersection <br /> Improvements at 58th Avenue and State Road 60 <br /> AMENDMENT NO . 4 TO THE PROFESSIONAL ENGINEERING/LAND SURVEYING <br /> SERVICES AGREEMENT BETWEEN KIMLEY - HORN AND ASSOCIATES , INC . AND <br /> INDIAN RIVER COUNTY , FLORIDA. <br /> This is an amendment to the existing Engineering/Land Surveying Services Agreement <br /> (AGREEMENT) dated February 22nd , 2005 between Kimley- Horn and Associates , Inc . <br /> (ENGINEER) and Indian River County (COUNTY) . This amendment addresses changes in <br /> " Section I — Project Limits and Description" , " Section III - Scope of Services " and " Section V - <br /> Compensation " of the AGREEMENT . <br /> Amendment Description <br /> This Amendment includes the following : <br /> " SECTION I - PROJECT LIMITS AND DESCRIPTION " is being modified to incorporate the <br /> following : <br /> Consultant services are required for the preparation of an interim roadway improvement <br /> plan for the 58 `h Avenue corridor . These services consist of supplemental survey and <br /> interim roadway improvement plans at the 58th Avenue/ State Road 60 intersection in <br /> unincorporated Indian River County , Florida . The interim roadway improvements <br /> identified by the COUNTY consist of the following : <br /> • Closure of the existing full access median opening located along 58t" Avenue <br /> between State Road 60 and College Lane . <br /> • Increase the vehicular storage capacity associated with the northbound dual <br /> left turn lanes at the State Road 60/ 58t Avenue intersection . <br /> • Design a southbound left turn directional median opening along 58th Avenue <br /> between State Road 60 and College Lane . This directional median opening <br /> shall be designed to accommodate a u-turn vehicular movement . <br /> " SECTION III — SCOPE OF SERVICES " is being modified to incorporate the following : <br /> B . Supplemental Desmon Survey <br /> The ENGINEER shall collect supplemental survey data along 58t" Avenue to <br /> depict the current access condition associated with the newly constructed <br /> commercial plaza located along the east side of 58t" Avenue between State Road <br /> 60 and College Lane .
