04• og• os. <br />g,L• <br />fir... ;_ ... •�Cf� <br />INDIAN RIVER COUNTY <br />66"` Avenue/ County Road 510 Roadway Widening <br />AMENDMENT NO. 4 TO THE PROFESSIONAL ENGINEERING/LAND SURVEYING <br />SERVICES AGREEMENT BETWEEN KIMLEY-HORN AND ASSOCIATES, INC. AND <br />INDIAN RIVER COUNTY, FLORIDA, <br />This is an amendment to the existing Engineering/Land Surveying Services Agreement <br />(AGREEMENT) dated February 22°d, 2005 between Kimley-Horn and Associates, Inc. <br />(ENGINEER) and Indian River County (COUNTY). This amendment addresses changes in <br />"Section I - Project Limits and Description", "Section III - Scope of Services", "Section V - <br />Compensation" and "Section XVI — Duration of Agreement" of the AGREEMENT. <br />Amendment Description <br />This Amendment includes the following: <br />"SECTION I — PROJECT LIMITS AND DESCRIPTION" is being modified to incorporate the <br />following: <br />Consultant services are required for the preparation of design surveys, right-of-way maps, <br />parcel sketches and legal descriptions, subsurface geotechnical investigations and <br />roadway improvement plans associated with planned improvements to the Schumann <br />Drive/ Barber Street intersection. <br />The intersection improvements shall consist of a single left tum lane, one through lane, <br />and one dedicated right turn lane on all approaches. In addition to the above describe <br />lane configuration, the northbound approach shall have an additional left tum lane (dual <br />northbound lei turn lanes) and two westbound receiving lanes on the eastbound <br />approach. Signalization improvements shall be designed to accommodate a mast -arm <br />signal pole configuration. <br />Based upon <br />the proposed <br />improvements described above the total <br />project length is <br />estimated to <br />be 6,600 linear <br />feet or 1.25 miles. <br />"SECTION III — SCOPE OF SERVICES" is being modified to incorporate the following: <br />B. Design Survey <br />The ENGINEER shall provide design survey services necessary to support and <br />facilitate contemplated design and permitting activities associated with this <br />project. This task will consist of the preparation of digital base map topographic <br />surveys in accordance with the intent of the Florida Minimum Technical <br />
