Indian River County, Amendment No. 4 <br />66" Avenue/ County Road 510 Roadway Widening <br />improvements at the Schumann Drive/ Barber Street intersection. Mast -arm <br />signal configuration will be designed to meet current COUNTY and FDOT <br />standards. <br />H. Structure Headwall Desian <br />The ENGINEER shall prepare preliminary and final plans associated with the <br />culvert extension and headwall replacement associated with the cross drain <br />culvert located west of Schumann Drive under Barber Street. <br />Culvert plans will be prepared associated with the culvert located along <br />the east leg of Barber Street. The design and construction documents will <br />be prepared utilizing FDOT's LRFD Culvert Program. <br />2. Construction and material specifications will be based on FDOT Standard <br />Specifications for Road and Bridge Construction, 2007 Edition. <br />3. All design and plan details will be in the English unit system. <br />"SECTION V — COMPENSATION" is being modified to incorporate the following: <br />The COUNTY agrees to pay and the ENGINEER agrees to accept for services rendered <br />pursuant to this Agreement fees inclusive of expenses in accordance with the following: <br />A. Professional Services Fee <br />The basic compensation mutually agreed upon by the ENGINEER and the <br />COUNTY is as follows: <br />Task Labor Fee <br />Lump Sum Components <br />Design Topographic Survey $23,700.00 <br />66th Avenue Additional Survey Services $165062.50 <br />Right -of -Way Survey and Mapping $26,250.00 <br />66th Avenue/ Schumann Drive Route Study $37,020.00 <br />Roadway Plans $82,480.00 <br />Signing & Marking Plans $95170.00 <br />Stormwater Design/ Environmental Assessment $199630.00 <br />Permitting $11,670.00 <br />Stormwater Pollution Prevention Plan $4,240.00 <br />Signalization Plans $8,770.00 <br />Structural Headwall Design $6,500.00 <br />
