IK <br /> d/d - 030 <br /> IMPROVEMENT OF 43RD AVENUE <br /> PROFESSIONAL CIVIL ENGINEERING & STRUCTURAL ENGINEERING <br /> SERVICES <br /> AMENDMENT NO . 2 TO WORK ORDER NO , 1 TO THE PROFESSIONAL <br /> ENGINEERINGILAND SURVEYING SERVICES AGREEMENT BETWEEN ARCADIS <br /> AND INDIAN RIVER COUNTY , FLORIDA. <br /> This is an amendment to the existing Engineering Services Agreement (AGREEMENT) dated <br /> September 6, 2005 between ARCADIS (ENGINEER) and Indian River County (COUNTY) . <br /> This amendment addresses changes in " Section I — Project Limits and Description" , " Section III - <br /> Scope of Services " , and " Section V - Compensation " . <br /> Amendment Description <br /> This Amendment includes the following : <br /> " SECTION I - PROJECT LIMITS AND DESCRIPTION " is being modified to incorporate <br /> the following : <br /> Consultant services are required for the preparation of road design plans ; bridge plans ; <br /> permit applications and utility relocations associated with the construction of 43 `d Avenue <br /> Improvements . The improvements consist of the following : <br /> Stormwater Drainage <br /> Work Order No l for the widening of 43 `d Avenue from Oslo Road to 8th Street addressed <br /> the potential need for storm water drainage ponds for the project . This work order noted <br /> that the design of drainage ponds may be required for the project, and stated that if <br /> drainage ponds were required , the preparation of plans for the ponds would be <br /> incorporated into the project through an amendment once the final drainage requirements <br /> were determined . ARCADIS has analyzed the need for and the number and location of <br /> drainage ponds required for the project . This Amendment No 2 incorporates the design of <br /> the drainage ponds into the project . <br /> The proposed storm water detention ponds will be constructed at the following locations : <br /> a " COUNTY STUMP DUMP SITE " located north of 5th Street SW , 500 feet west <br /> of 43 `d Avenue . <br /> • " MOOSE LODGE SITE " located 800 feet east of 43 `d Avenue between 4th Street <br /> and Ist Street SW . <br /> • "FORESTRY SITE" located at the northwest corner of 43 `d Avenue and 4th Street . <br /> • "DURWIN SITE" located 850 feet north of 4th Street along the west side of 43 `d <br /> Avenue . <br /> Page 1 of 5C \ Documents and Settings\hdeibekLocal Settings\Temporary Internet Piles\Content .
