Indian River County, Amendment No . 3 <br /> Oslo Road Widening from 43rd Avenue <br /> to east of 27'1' Avenue <br /> constructed within Oslo Road Phase I Improvements . A westbound right turn auxiliary <br /> lane will be included within the Oslo Road/ 27 `h Avenue intersection improvement . In <br /> addition , the roadway construction documents will be revised to establish the ultimate <br /> roadway signing and marking plan between 27th Avenue and 20th Avenue . <br /> The roadway construction plans shall be amended to delineate a 6 foot sidewalk along the <br /> south side of Oslo Road between the existing bridge immediately east of 43rd Avenue and <br /> the western most driveway to Oslo Road from the commercial development located in the <br /> southeast quadrant of the Oslo Road/ 43rd Avenue intersection . <br /> D . Drainage Design and Permitting_ <br /> 1 . Revise the drainage investigations and analysis along Oslo Road between 43rd <br /> Avenue and 27th Avenue necessary to resubmit a design which will drain the <br /> project in accordance with the County , St Johns River Water Management District <br /> ( SJRWMD ) , Indian River Farms Water Control District (IRFWCD ) and Army <br /> Corps of Engineers (ALOE) design criteria . <br /> In addition , the existing Oslo Road Phase I SJRWMD ERP shall be modified to <br /> include the addition of the westbound right turn auxiliary lane to the intersection . <br /> 2 . The ENGINEER shall prepare stormwater permit applications for the SJRWMD , <br /> IRFWCD and ACOE for submittal by the COUNTY . This will consist of all <br /> required evaluation , design , coordination , and follow - up work necessary to <br /> support permit applications . The COUNTY will review the permit applications as <br /> necessary . The ENGINEER shall assemble and be responsible for the final <br /> submittal . <br /> 3 . The Stormwater Pollution Prevention Plan (SW3P) will be revised in conjunction <br /> with this amendment. The site specific SW3P is a requirement of both the EPA <br /> National Pollutant Discharge Elimination System (NPDES ) General Permit for <br /> Stormwater Discharges from Construction Activities , and the FDEP Generic <br /> Permit for Stormwater Discharges from Construction Activities . <br /> 4 . All permit fees will be paid directly by the COUNTY and are not included in the <br /> lump sum fees . Any significant plan revisions caused by changing agency criteria <br /> after our initial design is reviewed with each appropriate agency and similar <br /> factors outside of ENGINEER ' S control , additional meetings or coordination will <br /> be considered as Additional Services . <br /> E . Si nalization Desi <br /> Revise the signalization design located at the Oslo Road and 27th Avenue intersection to <br /> provide for the future lane additions associated with 27 `h Avenue . The 27 `h Avenue lane <br /> additions consist of dual left turn auxiliary lanes , two through lanes and dedicated right <br /> turn auxiliary lane in both the northbound and southbound approaches . <br /> Page 2 of 5 <br />
